Forms are not sold online! Forms are only obtainable at the school's account Office 

Forms are obtainable from the School Account's Office on presentation of a teller evidencing payment of a non-refundable fee of N20,000:00 ONLY for

• Basic Nursing 
• BASIC Midwifery   And 
• Post Basic Nursing

Entry Requirement:

• Five (5) Credits in the following subjects: English language, Mathematics, Chemistry, Physics and Biology in not more than two sittings in WAEC, NECO,GCE.

NOTE: Completed forms should be submitted to the school examination office on or before 26th March, 2024
